I will create a web contact form for your website with up to 10 fields. The contact form will be delivered directly to your inbox every time someone fills it out. The contact form is comprised of several files and will be delivered in a ZIP file. Just unzip and it's ready to upload into your site. Note: Plain Text FIelds only.
