I will build a custom HTML form with validation and PDF download

About this gig
Replace a paper or PDF form with an easy browser form. Type the details, check any errors and download a neatly formatted PDF.
Useful for equipment handovers, repair intake and simple checklists.
For $35 you receive:
- One custom form with up to 15 agreed fields
- Date, quantity, GBP amount and email-format checks
- PDF download and save/reopen for unfinished drafts
- Your wording, simple branding and source file
- Short instructions, 3-day delivery and one revision
Open the supplied HTML file in Microsoft Edge on Windows. It works locally: no hosting, account or subscription. Entries are not sent to a server.
Message me on Fiverr with a blank, non-confidential form and the checks you need. I will confirm the scope before you order.
The PDF is a completed record; edit details in the browser form or saved draft. No online submissions, databases, payments, logins or e-signatures. Complex layouts and other devices/languages need a separate scope check. Email checks cover format, not whether the address exists.
I use AI-assisted tools to build and check the form. Keep messages, files and payments on Fiverr.

FAQ
What do I open and how do I use it?
Open the supplied START HERE HTML file in Microsoft Edge on Windows. Type into the boxes, then click Download completed PDF. Choose Save draft if you want to keep an editable copy to reopen later. Short instructions are included.
Do I need a website, account or subscription?
No. This package is a local browser form. It does not publish a website, collect online submissions or send entries to a server. It creates files on the computer of the person filling it in.
What can the form check?
We agree each field's rules before ordering: required text, real dates, whole quantities, pound amounts, common email formats and checkboxes. Phone numbers and references keep their starting zeroes. Checks do not establish someone's identity or whether an email address exists.
