About this gig
Tired of printing, filling by hand and scanning forms? I'll turn your Word file, Excel sheet, scanned paper or existing PDF into a clean, fillable PDF form that works on screen.
I'm a Fiverr Pro seller with 125 five-star reviews, and I build forms in Adobe Acrobat Pro and PDF-XChange Editor Pro.
What I can add to your form:
- Text fields, checkboxes, radio buttons and dropdowns
- Automatic calculations (totals, tax, discounts, dates)
- Field validation and required fields
- Fields that show or hide based on answers
- Digital signature fields
- Print, reset, email or submit buttons
- Correct tab order so it's fast to fill in
Great for invoices, order forms, applications, checklists, contracts and internal company forms.
Need stamps too? I also create dynamic PDF stamps with date, time, signatures, seals and approval status. See my PDF stamp Gig.
Message me with your form before ordering and I'll recommend the right package.

FAQ
What files can you convert into a fillable form?
Word, Excel, existing PDFs, and scanned paper forms. For scans, a clear, straight image gives the best result.
Will my form work in any PDF reader?
Forms work best in Adobe Acrobat or Reader and PDF-XChange. Calculations and scripts may not work in web browsers or some mobile apps. Tell me what your users open it with and I'll test for that.
Can you add automatic calculations?
Yes. Totals, tax, discounts, date math and custom formulas based on what the user enters.
Can you redesign the look of my form?
I can clean up layout and alignment. Full graphic redesign can be discussed before ordering.
Do I get the editable file?
Yes. You get the final fillable PDF, and you own it.
