No. Prompting is one possible finding among several. More often the break is in the handoff between steps, or in the absence of any check at all. I report what I actually find.
We use several different AI tools. Does that matter?
No. I look at the workflow, not the vendor. The failure patterns are remarkably consistent across tools.
