One clearly described problem with one cause. If your form is broken on mobile and your checkout errors out, that is two bugs.
What do you need from me?
The URL where it happens, what you expect, what happens instead, and access to the site or CMS. A screenshot or short screen recording helps a lot.
What if you cannot fix it?
Then I tell you that before you order. Send me the details first - I give you an honest answer for free.
