I will audit your behavioral health documentation for compliance risks
About this Gig
Documentation gaps can lead to corrections, payment delays, or audit findings. I provide nonclinical reviews of de-identified behavioral health progress notes to help providers identify errors and improve audit readiness.
Depending on the package, I assess:
Required elements and internal consistency
Time, location, signatures, and credentials
Whether documentation supports the recorded service
Individualization and documentation of medical necessity
Alignment with buyer-provided policies or audit criteria
You will receive an organized risk report with findings and practical recommendations. Standard includes trend analysis across the sample. Premium adds a prioritized action plan, custom audit checklist, and 30-minute consultation.
I bring more than five years of behavioral health quality assurance, high-volume documentation auditing, operations analysis, and staff training experience.
Only de-identified records may be submitted. Each order covers one program, service type, and requirements set. This service is not legal, clinical, or certified coding advice and does not guarantee payment or audit outcomes. Contact me before ordering state- or payer-specific research.

FAQ
What types of records do you review?
I review de-identified behavioral health progress notes and service documentation. Supporting policies, templates, and buyer-provided audit criteria may also be included when relevant.
Can I submit records containing patient information?
Do not upload protected health information to Fiverr. If de-identification is not feasible, contact me before ordering. Identifiable records can only be considered through an approved HIPAA-compliant workflow after the required agreements and safeguards are in place.
How is the sample size determined?
Basic reviews 25 notes from one provider as an initial risk screen. Standard reviews 60 notes from one provider as an expanded audit. Premium reviews up to 150 notes from one provider. Different volumes or audit objectives may require a custom offer.
How should the notes be selected?
Notes should be randomly selected across the complete review period and should represent the provider’s applicable dates, services, and codes. Avoid selecting only records already believed to be correct or incorrect.
What does the documentation review evaluate?
The review may evaluate required elements, consistency, signatures, credentials, time, location, service documentation, individualization, documentation supporting medical necessity, and alignment with buyer-provided requirements.
Do consulting hours mean live meeting hours?
No. Consulting hours represent the estimated time spent reviewing records, analyzing findings, and preparing deliverables. Premium includes one 30-minute consultation.
Do you guarantee compliance, reimbursement, or audit results?
No. This service identifies documentation risks and improvement opportunities but cannot guarantee regulatory compliance, payer approval, reimbursement, or audit outcomes.
What does a revision include?
A revision includes clarification or correction of the delivered report. New records, changed audit criteria, additional providers, or an expanded review period are not revisions.
Should I contact you before ordering?
Yes, if your project involves multiple providers, multiple service types, treatment or service plans, notes longer than three pages, a different record volume, or state- or payer-specific research. Message me through Fiverr so I can recommend a package or custom offer.
