Delegated or application permissions?
Delegated acts as a signed-in user; application acts as your service with no user present. I'll recommend the right one for your case during scoping.
Can you get real-time updates instead of polling?
Yes, via Graph change notifications. That's the Premium package.
Does this work with personal Outlook.com accounts?
Sometimes, but personal accounts support a narrower set of Graph functionality than work or school accounts. Ask me first.
