This is a Project Risk Log (or Risk Register) is built in Excel. It has fields to log your Risks, the Probability, Impact, Trigger Event, Mitigation and Contingency Plans for each. It has conditional formatting to color code high ranking risks and a macro to bring the highest ranking risks to the top of the list. The Risk Matrix (see image for this Gig) quickly summarizes how risky this project is via a picture. This aligns to PMI's "Probability and Impact Matrix" as defined on page 317 of PMBOK 5th edition (2013).
